At the end of each year, all service and facility dog applications expire.  By completing the form below, you will renew your application and remain in our applicant pool.

IMPORTANT: Please note, although our applicant screening process remains the same, TLCAD has updated our Team Training process, so we are able to train and place more service dogs with those in need. Prior to completing the below form, APPLICANTS MUST review the updated requirements HERE.
