Furlough Feedback Form

Thank you so much for furloughing a TLCAD dog!  The opportunity to relax and experience a home environment is an important part of service dog training.  To assist our trainers with ongoing training, please answer the following questions as accurately as possible.  Put a checkmark if you noticed the dog engaging in the behavior, leave the box blank if you did not notice the behavior.  Please provide specific details in the “comments” sections.  The more information we can provide to the trainers, the more effective the dog’s training will be.  Thank you!
